Experiencing headache symptoms after sinus surgery is not uncommon. You might find yourself wondering, “Is it normal to have bad headaches after sinus surgery?” or “How long do headaches last after sinus surgery?” While some temporary pain or discomfort can be expected due to inflammation and healing, severe or persistent headaches warrant further investigation, especially frontal sinus pain.
This article explores potential causes of headaches after sinus surgery, including sinus pressure after septoplasty, and how an ENT specialist can help. It also discusses how septoplasty relief can be achieved in some cases.
Case Study: Headaches and Sinus Pain After Surgery
Madam A, a patient of Dr Gan, underwent sinus surgery for recurrent sinusitis. However, a month later, she developed headaches in her left forehead region, a symptom she didn’t have before.

Examination revealed scarring near the drainage pathway of her left frontal sinus, leading to complete blockage and frontal sinus pain.

Madam A underwent revision surgery using the Image Guidance System (IGS) to unblock her frontal sinus. IGS helps surgeons to determine the exact position of the surgical instrument during sinus surgery. The position of the surgical instrument is indicated by a green cross and superimposed on the CT scan images (refer to images below).

This is especially useful in revision sinus surgery, where scarring or loss of surgical landmarks caused by previous surgery may make it challenging for the operating surgeon. Another good use of IGS is in patients with complex sinus anatomy (especially in the frontal recess).
During Madam A’s sinus surgery, a thick mucous was discovered in the left frontal sinus. The frontal sinus pathway was widened and flushed with normal saline.

After the revision surgery, Madam A’s frontal sinus pain resolved. This case highlights the importance of meticulous surgical technique and the potential for complications, even in seemingly straightforward cases.
Understanding Headaches After Sinus Surgery
Headaches after sinus surgery can arise from various factors:
1. Inflammation and Healing
Sinus surgery typically involves manipulating delicate tissues, which can lead to inflammation and pain. This pain after sinus surgery is usually temporary and subsides as healing progresses.
2. Scar Tissue Formation
Scar tissue can form during the healing process, potentially obstructing sinus drainage pathways and causing sinus pain after surgery. This can lead to pressure buildup and headaches.
3. Pre-existing Conditions
Patients with pre-existing headache conditions, such as migraines, may experience headaches after sinus surgery due to the stress of surgery and recovery.
4. Other Factors
Other factors that can contribute to headaches after sinus surgery include:
- Medications: Certain medications used during or after surgery can cause headaches as a side effect.
- Dehydration: Insufficient fluid intake can worsen headaches.
- Stress and anxiety: The stress of surgery and recovery can trigger headaches in some individuals.
Managing Headaches After Sinus Surgery
If you experience headaches after sinus surgery, especially frontal sinus pain or sinus pressure after sinus surgery, consult an ENT specialist in Singapore. They can help evaluate the cause and recommend appropriate management strategies.
Seek medical attention if:
- Headaches are severe or persistent.
- Pain after sinus surgery is not improving with time.
- Headaches are accompanied by other symptoms, such as fever, nasal discharge, or vision changes.
- You are concerned about your recovery.
That said, treatment for headaches after sinus surgery typically depends on the underlying cause. Options may include:
- Pain medication: Over-the-counter pain relievers can help manage mild to moderate pain.
- Nasal irrigation: Rinsing the nasal passages with saline solution can help reduce inflammation and clear mucus.
- Nasal steroids: These medications can reduce inflammation in the nasal passages.
- Antibiotics: If a bacterial infection is present, antibiotics may be prescribed.
- Revision surgery: In some cases, revision surgery may be necessary to address complications such as scar tissue formation or blockage of the frontal sinus.
Recovery Time After Sinus Surgery
Recovery time after sinus surgery varies depending on the extent of the procedure and individual healing. Most patients experience significant improvement within a few weeks. However, complete healing can take several months.
If you’re experiencing headaches 3 weeks after sinus surgery or later, it’s advisable to discuss this with an ENT doctor. They can assess your progress and recommend further management if needed.
By understanding the potential causes of headaches after sinus surgery and seeking timely medical attention, you can optimise your recovery and achieve long-term relief.
